Root cause analysis is a problem solving process for conducting an investigation into an identified problem, concern, incident or non-conformity. This is a completely separate process to incident management and immediate corrective action. RQA can either assist you by carrying this process out on your behalf or by training your people in root cause analysis.
Root Cause Analysis Investigation
Following an incident RQA’s expert consultant can assist you in investigating the root cause of the incident. During a root cause analysis investigation, the consultant will look further than the solution to the immediate problem and gain an understanding of the fundamental or underlying causes and will assist in correcting them, with the aim of preventing re-occurrence of the same issue. This could involve the identification and management of processes, procedures, activities or indeed inactivity, behaviour or prevailing conditions.

RQA can also train your teams to be able to carry out root cause analysis themselves in order to identify permanent solutions, prevent recurring failures, and apply a logical problem solving process applicable to any identified problem, concern, incident or non-conformity.
